In My ASU, click Benefits in the My Employment box. You will see a link to My Benefits Summary. Please print this page and upload it to the J-1 Annual Benefits Summary Upload eForm through the ISSC eForm portal.
As a reminder, the U.S. Department of State has the following medical insurance requirements for J-1 scholars and their J-2 dependents.

| Medical evacuation | $50,000 |
| Repatriation of remains | $25,000 |
| Deductible | Not to exceed $500 per accident or illness |
